This image is real. It's leaked concept art from ハーフ - 生活 : レジの下の愛, an officially licensed Half-Life spin-off to be created by Japanese game company Spike Chunsoft, known for their work on DANGANRONPA: Trigger Happy Havoc. While the name translated from Japanese was meant to be 'HALF - LIFE: LOVE UNDER REGIME', it was to be known in the west as HALF-LIFE 2: THE VISUAL NOVEL.
5 years into development, the entire project was scrapped and every Valve employee who had anything to do with it was fired, all of them mysteriously ending up dead weeks later. Gabe Newell, founder and CEO of Valve, refuses to comment on their disappearances and soon murders.
It is speculated that, despite Valve's non-pornographic record, the game would feature sex scenes in featuring the characters. This is later backed by files and folders in the leaked source code titled 'HSCENES', the H likely standing for hentai. Alas, it seems the game will never be released as Valve has gotten far stricter over the years for what games are released or not by their quality standards.

EDIT: From some more scrubbing of the game files, a 'stealth' mechanic was revealed, supposedly to hide your love among citizens of City 17 from Combine forces to avoid prosecution.
